Memorial Fund and Support Program

I    Title            DHS Class of 1956 Memorial Fund & Support Committee

II.  Purpose   Acknowledge classmate, surviving spouse, or life partner who is seriously ill or has passed away.

     A.  Death

          i.          Offer assistance to the family (if requested) in the planning and execution of a Memorial Service.
          i.i.        Order floral arrangement with DHS Class of 1956 ribbon, or accommodate appropriate acknowledgment
                      to surviving spouse, life partner, or children (if floral arrangements are discouraged by the family).
          i.i.i.      Accommodate surviving family’s requests for memorabilia that is available for loan.

     B.  Illness

          i.          Send appropriate cards to a classmate, spouse, or life partner who is hospitalized or convalescing.
          i.i.        Call or visit classmate if hospitalized or convalescing within Greater Portland area, if appropriate.

III.  Donations

     A.  Funds

          i.          Contributions shall be voluntary.  Amount shall be at the donor’s discretion.
          i.i         Memorial Fund contributions may remain anonymous at donor’s request.
          i.i.i.      Donations may be made annually, or at the time of a classmate’s death.
          i.i.i.i.    Payments shall be made payable to the DHS 1956 Memorial Fund and sent to Fund Coordinator.
          i.i.i.i.i.  Treasurer shall credit contributions to the DHS 1956 Alumni Memorial Fund

     B.  Donation Form

          i.          Submit donations with form located at http://www.dhs1956.org/forms/form_dues_memorial_fund.pdf
                      Form also enclosed within newsletters mailed via USPS or available from Fund Coordinator.

IV. Budget  (Guidelines based upon available funds)

     A.  Funeral, Celebration of Life, Memorial Services

          i.          Floral arrangement with DHS Class of 1956 ribbon or appropriate acknowledgement to the surviving spouse, life
                      partner, or surviving children shall not exceed $100.
          i.i.        Condolence Cards to surviving spouse, or life partner, and surviving family. Expense at committee’s discretion.

     B.  Hospitalization, Convalescence. Compassion

          i.          Appropriate Card to classmate, surviving spouse, or life partner. Expense at committee’s discretion.
          i.i.        Call or visit classmate if hospitalized or convalescing within Greater Portland area if appropriate. No expense.
